You have three questions here, so I’ll answer each in term. Question #1, in terms of keywords and your home page, the answer is: YES. Define your target keyword or keyphrases, and put them strategically on your home page, starting with the all-important TITLE tag. The TITLE tag of the home page remains the most important position on any website, so think carefully what keywords you put into that TITLE tag, and remember it needs to be less than about 80 characters. All other content on the home page should be keyword-heavy, and as I teach in my classes on SEO, make sure that you use your home page as a ‘one-click’ gateway to your target landing pages.
As for your second question, the use of keywords in image tags, that remains important, too. What you want to do is establish the keyword target of a given page, and make sure you have at least one image on that page with the ALT attribute defined to be the target keyword. In this way, your image ALT reinforces the visible keyword-heavy content on the page, including the TITLE and other strategic tags such as the H1 tag.
Finally, as for your third question, does SEO work well for medical professionals, it all depends. If you have patients / prospects who tend to use Google (e.g., searches for knee doctor, plastic surgeon and the like), then YES absolutely Google matters. Third-party sites like zocdoc are only relevant to the extent that patients truly use them. In my experience, most people go FIRST to Google and then might use reference sites to ‘verify’ that a Dr. is good, but few people start their searches there. But it is ultimately not an either-or question, but probably a ‘both’: do BOTH SEO for Google and use review sites that are specific to your medical specialty, with a cynical eye to which sites patients ACTUALLY USE.
