First, for the ‘short tail’ you have localization at work. When you type in ‘knee surgeon,’ Google ‘infers’ that this is a local search and (secretly) appends your location to it, ‘as if’ you searched ‘knee surgeon san francisco.’
Secondly, for searches that have a local character, many times the search then becomes very focused on Google+ Local which is DIFFERENT than regular SEO because REVIEWS are very important.
The take-away here is to try to research how different customers search and then pay attention to WHICH GAMES are at place (Just SEO? Just Google+ Local? Both SEO AND Google+ Local), and then what are the RULES of that game. For searches in which Google+ local is important you have to get REVIEWS. REVIEWS are a major, major factor there, whereas in ‘traditional’ SEO that is not the case.
