Answer: Keyword stuffing is putting your keywords over and over and over. There are really crude versions of it like keyword stuffing, keyword stuffing, keyword stuffing, keyword stuffing, keyword stuffing, keyword stuffing, keyword stuffing, keyword stuffing, keyword stuffing, keyword stuffing, keyword stuffing, keyword stuffing, (comma comma comma) phrases. More sophisticated ways are just repeating the phrase strategically in various tags.
There is a fine line between keyword stuffing and using keywords in an appropriately dense way; as I teach my students, we DO want keyword heavy content for Google, just not to overdo it. As for your second question, ONCE you know your keywords, then you strategically place them in your TITLE tags, etc., as I teach in the class on page tags.
Knowing where to put them is a HUGE part of SEO.
